
The Blueprint to End Homelessness recommends a “System of Care” approach for all homeless populations, including those seriously at risk for homelessness but not yet technically in that status. This approach calls for the organization and delivery of care through individual service plans, designed and organized to address the specific needs, goals, challenges, and risks of individual clients.
These service plans are more than a screening and referral process to separate programs and service providers. They include a comprehensive assessment across all relevant life domains, with integration of all housing, clinical, and social support needs and challenges into a single set of goals, challenges, and strategies to support sustainable community living. Service plans are developed, organized, and coordinated by a single care manager, who works closely with the client and family to overcome challenges and achieve individual goals. The Blueprint offers a powerful vision of a system that coordinates individualized housing arrangements with organized services and resources through a single service plan with common goals and integrated strategies.
