Plan to End Chronic Homelessness

We believe individuals experiencing chronic homelessness have the right to be housed and connected to care.

CHIP and the Indianapolis Continuum of Care are responsible for developing and implementing the Community Plan to End Chronic Homelessness. In conjunction with the Indianapolis Community Plan to End Homelessness, the plan for chronic homeless identifies strategies and action steps that address the unique needs of individuals and families experiencing homelessness in our community, including those facing issues related to substance-use, mental or physical health, or re-entry from incarceration.
